Description.

Provide specialist palliative and end-of-life care pharmacy services across NHS hospitals, hospices, and community palliative care teams. Support symptom control, optimise complex medicines regimens, contribute to multidisciplinary care planning, and ensure safe use of high-risk analgesics and end-of-life medicines.

Key Requirements.

Experience.

  • Minimum 1–2 years’ post-registration experience in palliative care, end-of-life care, oncology, or advanced clinical pharmacy.

  • Skilled in managing analgesics, syringe drivers, anticipatory medicines, and complex symptom-control regimens.

  • Experience working closely with palliative care consultants, CNSs, hospice teams, and community specialist nurses.

  • Familiar with NICE guidance, local palliative care formularies, and end-of-life medicines pathways.

Qualifications.

  • MPharm degree (or recognised equivalent) with full GPhC registration.

  • Postgraduate diploma or certificate in Clinical Pharmacy desirable.

  • Additional palliative care modules or advanced symptom-management training advantageous.

  • CPD in palliative therapeutics, pain management, and end-of-life care.

Compliance and Vetting.

  • Enhanced DBS on the Update Service.

  • Full identity and right-to-work verification.

  • Two recent professional references.

  • Up-to-date NHS Statutory and Mandatory Training.

  • Immunisation compliance for hospital and community clinical environments.

  • Full GPhC registration and fitness-to-practise check.

  • Verification of qualifications and employment history.
